SUSTAIN

The SUSTAIN project is a four year initiative (2015-2019) funded through the European Commission Horizon 2020 Work Programme 2014-2015; Health, demographic change and wellbeing.

Improve integrated care

The project aims to improve integrated care for older people living at home with multiple health and social care needs. The SUSTAIN project will work with two existing integrated care sites in seven European countries and will support the sites in further improving their care based on a number of identifiers.

The SUSTAIN project includes 9 partners from European countries—The Netherlands (Vilans, VUMc, RIVM), UK, Norway, Belgium, Austria, Spain, Estonia, Germany – who will work with sites as well as additional European partners – Ireland and Belgium – who will support the project.
